Cognito Health Inc. Privacy Policy
Cognito Health Inc. (“Cognito”) or (“us”). This Privacy Policy describes how your personal information is collected, used, and shared when you visit www.getcognito.ca (the “Site”).
The headings used in this Privacy Policy are included for convenience only and will not limit or otherwise affect this Policy.
SECTION 1.A - INFORMATION WE COLLECT
When you visit the Site, we automatically collect certain information about your device, including information about your web browser, IP address, time zone, and some of the cookies that are installed on your device. Additionally, as you browse the Site, we collect information about the individual web pages that you view, what websites or search terms referred you to the Site, and information about how you interact with the Site. We refer to this automatically-collected information as “Device Information”.
With explicit consent, we may collect Personal Information from you such as your email address and/or phone number when you request information about our Services, take a self-assessment, register for our newsletter, request customer or technical support, or otherwise communicate with us. Additionally when you make a purchase or attempt to make a purchase through the Site, we collect certain information from you, including your name, payment information (including credit card numbers), and email address. We refer to this consensually collected information as “Personal Information”.
SECTION 1.B - HOW WE COLLECT INFORMATION

In addition, we use a 3rd party marketing tool called Hotjar to better understand our users' needs and to optimize this service and experience. Hotjar is a technology service that helps us better understand our users' experience (e.g. how much time they spend on which pages, which links they choose to click, what users do and don't like, etc.). This enables us to build and maintain our service with user feedback. Hotjar uses cookies and other technologies to collect data on our users' behavior and their devices. This includes a device's IP address (processed during your session and stored in a de-identified form), device screen size, device type (unique device identifiers), browser information, geographic location (country only), and the preferred language used to display our website. Hotjar stores this information on our behalf in a pseudonymized user profile. Hotjar is contractually forbidden to sell any of the data collected on our behalf.

SECTION 2 - HOW DO WE USE YOUR PERSONAL INFORMATION AND DEVICE INFORMATION?
We use your Personal Information for communicating directly with you. Examples may include:
-Responding to your questions, comments, and other requests.
-Providing access to certain areas, functionalities, and features of our Site and Services.
-Processing your financial information and other payment methods for Services purchased.
-Processing your applications and transactions.
-Answering your requests for customer or technical support.

We use Mailchimp to allow us to communicate with you via email. We always ask for explicit consent before sending your email address to Mailchimp.
Finally, we may also share your Personal Information to comply with applicable laws and regulations, to respond to a subpoena, search warrant or other lawful request for information we receive, or to otherwise protect our rights.
SECTION 4 - BEHAVIOURAL ADVERTISING
As described above, we use your Personal Information & Device Information to provide you with targeted advertisements or marketing communications we believe may be of interest to you.

If you are a European resident, you have the right to access Personal Information we hold about you and to ask that your personal information be corrected, updated, or deleted. Additionally, if you are a European resident we note that we are processing your information in order to fulfill contracts we might have with you, or otherwise to pursue our legitimate business interests listed above. Additionally, please note that your information will be transferred outside of Europe, including to Canada and the United States.

When you make a purchase through the Site, we will maintain your Personal Information for our records unless and until you ask us to delete this information.

The Site is not intended for individuals under the age of 17.

We may update this privacy policy from time to time in order to reflect, for example, changes to our practices or for other operational, legal or regulatory reasons.

At Cognito we take privacy and security very seriously. Protecting your Personal Information is of utmost importance to us and we use the industry leading tools to do so. This includes but is not limited to 2 factor authentication and role based access for staff, penetration testing of our applications on a recurring basis, thorough code review and quality assurance, and much more.
